Please notify the office if your child's health status has changed.
IMPORTANT: The following foms will need to be read and signed online. Please go to myDSD, login to your guardian account and sign each form. Your child will also need to login to their myDSD account (using their Username and PIN) and sign these forms.

NEW STUDENT REGISTRATION
New students must complete ALL of the registration forms for enrollment. Please print the NEW STUDENT CHECKLIST to assist you with registering. Some forms can be signed online and some must be printed, filled out and returned.
When registering - bring the following to the office at Valley View Elementary:
- Your child's original Birth Certificate
- Your child's immunization records from your health provider
- Your picture ID
- Proof of Residency (see below)

Health: On the Student Information Form under "Physical Status of Student" indicate your child's health status and any health problems that may exist.

Back to School Supplies 2019-20
Message from Mrs. Johnston
Each year we provide a list of optional supplies parents may purchase and donate to their child's classroom. You may donate any of the items listed to help with your child's classroom this school year. You might find that each grade level's list is slightly different as each grade level has different needs. You may donate multiple needed items if you wish.
All children will be provided with the same supplies whether they choose to donate or not.
Please have your child bring any donations with them on the first day of school and give them to your child's classroom teacher.

*Just a reminder that in its 2012 Legislative Session, the Legislature allowed for elementary school teachers to compile and provide to a student’s parent a suggested list of supplies for use during the regular school day so that a parent may furnish on a voluntary basis those supplies for student use. If a teacher chooses to do this, the list must include the following language: “Notice: The items on this list will be used during the regular school day. They may be brought from home on a voluntary basis. Otherwise, they will be furnished by the school.”
This language can be found in District Policy 6F-101 School Fees, Fee Waivers and Provisions in Lieu of Fee Waivers Section 2.1.5.
