Here you'll find the information and documents you need to make yearly registration easier. The documents are also available in the school office if you do not have access to a computer or printer. If you have questions please call 801-402-2050 and we will be happy to assist you.

Kindergarten & New Students
NEW STUDENT REGISTRATION
The following are the forms and documents that you will need to register your child at Valley View Elementary (Davis School District).
Some forms can be signed online and some must be printed, filled out and returned.
Paying for School Meals, or other Fees, Online -Instructions

1. To register a new student / new kindergarten student or a
returning student from Davis Connect, a charter, homeschooling, or another elementary school please first complete the online registration form below:

2. When you come to the Valley View office to REGISTER-
please bring the following required documents:
- Proof of Residency (see below)
- Your child's original Birth Certificate
- Your child's immunization records from your health provider - REQUIRED IMMUNIZATIONS
- Your picture ID
- All of the REQUIRED registration forms below that need to be printed, filled out and/or signed.
3. REQUIRED Forms
Print the following forms, fill out and return to Valley View Elementary:

Health: On the Student Information Form under "Physical Status of Student" indicate your child's health status and any health problems that may exist. -

4. REQUIRED Online Forms
IMPORTANT: Go to myDSD.davis.k12.ut.us, login to your parent/guardian account and fill out the required forms.
Your student will go to their myDSD and do the same.

Returning Students
RETURNING STUDENT REGISTRATION
ALL students, who are returning to Valley View Elementary, must have these REQUIRED forms completed by the first Friday of every school year.
School Meals are not free this year--if you would like to apply for free or reduced
meals go to myDSD.

Please notify the office if: your child's health status has changed or there need to be changes on your Student's Form

Back to School Supplies 2022-23
Back to School Supplies 2022-23
We provide all the materials and supplies needed in Elementary School. If you would like to donate supplies to your child's classroom please see the list of optional supplies below.
You might find that each grade level's list is slightly different, as each grade level has different needs. You may donate multiple needed items if you wish.
All children will be provided with the needed supplies whether they choose to donate or not.
Please have your child bring any donations with them on the first day of school and give them to their classroom teacher.
