Returning Students
RETURNING STUDENT REGISTRATION
ALL students, who are returning to Valley View Elementary, must have these REQUIRED forms completed by the first Friday of every school year.
REQUIRED
Please notify the office if your child's health status has changed.
IMPORTANT: The following foms will need to be read and signed online.
Please go to myDSD, login to your guardian account and sign each form. Your child will also need to login to their myDSD account (using their Username and PIN) and sign these forms.

The following form will need to be printed, signed, and returned to your child's teacher by the first Friday of the school year.
Every child must return this form every year.

-this form needs to be filled out yearly if your address is a temporary living arrangement due to loss of housing or economic hardship.

Kindergarten & New Students
NEW STUDENT REGISTRATION
New students must complete ALL of the registration forms for enrollment. Please print the NEW STUDENT CHECKLIST to assist you with registering. Some forms can be signed online and some must be printed, filled out and returned.
When registering - bring the following to the office at Valley View Elementary:
- Your child's original Birth Certificate
- Your child's immunization records from your health provider
- Your picture ID
- Proof of Residency (see below)
- All of the REQUIRED registration forms below that need to be printed, filled out and/or signed.

Back to School Supplies 2020-21
Back to School Supplies 2020-21
We provide all the materials and supplies needed in Elementary School. If you would like to donate supplies to your child's classroom please see the list of optional supplies below.
You might find that each grade level's list is slightly different, as each grade level has different needs. You may donate multiple needed items if you wish.
All children will be provided with the needed supplies whether they choose to donate or not.
Please have your child bring any donations with them on the first day of school and give them to their classroom teacher.
